Titanium anodising changes the surface colour through an electrochemical process without adding any coating that can wear off, while Surgical Steel pieces may use a black coating instead. Titanium doesn't carry the trace nickel found in Surgical Steel, making it the better option if you have sensitive skin. Both hold their black finish well with normal wear.
